Role of repeat transurethral resection of bladder tumour in patients with high grade non muscle invasive bladder cancer - A prospective observational study

Abstract:

Aim: To assess the role of repeat transurethral resection of bladder tumour in management of patients with high grade non-muscle invasive bladder cancer. Material and methods: The prospective observational study was conducted at Department of Urology at MMIMSR, Mullana, Ambala from - June 2023 to June 2025 among 50 patients with HG-NMIBC detected after TURBT. Data of all patients with HG-NMIBC detected after TURBT were assessed for detailed history, investigations, intra-operative and histopathological examination findings. RETURBT was performed 2-6 weeks following primary TURBT. Cystoscopy was done under general anesthesia with 20 Fr 30 degree cystoscope. Presence of malignancy in RETURBT surgical specimen was defined as residual tumour. Any progression in stage or grade during RETURBT was defined as tumour upstaging. Results: Residual tumour was detected in 13 patients (26%). Three patients had their disease upstaged from T1HG to T2 after which they were offered radical cystectomy. The remaining 10 patients had same disease stage at RETURBT as the primary resection. Residual tumour was found to be significant more in subjects with sessile, multifocal, carcinoma in situ, absence of muscle in specimen and having presence of lymphovascular invasion. RETURBT upstaged (to T2) were not significantly associated with absence or presence of muscle in specimen as well as lymphovascular invasion. Conclusion: RETURBT improves the staging of BC and may change the plan of management in certain patients. In future, it may be recommended that risk factors be analysed that further narrow the indications for RETURBT, for which larger, multi-centre studies will be required.
