The safety and efficacy of supine mini percutanous nephrolithotomy: A single center experience

Abstract:

Background: The supine position improved airway control, patient comfort, spontaneous fragment evacuation, and lower renal pelvis pressure. A modified supine-lithotomy position allows dual access, potentially enhancing rates of complete stone clearance and reducing operative time. This research was conducted to determine the clinical utility and safety of supine mini percutaneous nephrolithotomy.
Methods: This single center investigation was carried out on 40 patients of both sexes, presenting with moderate-sized renal calculi (1–3 cm) suitable for supine mini-percutaneous nephrolithotomy.
Results: Complications were observed in six patients (15.79%). Three patients experienced colicky abdominal pain requiring prolonged analgesic use beyond the second postoperative day. Two participants developed a fever exceeding 38 °C, which was treated with antipyretics and antibiotics. One participant re-admitted two weeks postoperatively due to hematuria and was treated with bed rest, administration of coagulants, and blood transfusion.
Conclusions: Mini-PCNL procedures in supine positions exhibited efficacy in treating medium sized renal stones. Notably, the supine approach resulted in a notably shorter duration of surgery and few complications, potentially offering an advantage for patients and surgeons.
