Comparative study of vesico-vaginal fistula repair with and without omental patch

Abstract:

Vesico-vaginal fistula (VVF) is a significant and distressing condition that leads to continuous urinary leakage into the vagina, severely impacting a woman's quality of life. Surgical repair is the primary treatment, and various techniques have been proposed to improve outcomes. This study aimed to compare the outcomes of VVF repair with and without interposition of an omental patch, an approach that has been suggested to promote healing and prevent recurrence. Conducted at Dhaka Medical College Hospital from March 2022 to August 2023, this quasi-experimental study included 50 patients diagnosed with VVF, who were allocated into two groups: Group A (repair with omental patch) and Group B (repair without omental patch). The mean age of patients was 34.80 ± 9.78 years in Group A and 34.48 ± 9.68 years in Group B, with similar etiology in both groups. Surgical time was comparable between the groups (93.2 ± 4.76 minutes in Group A and 92 ± 4.08 minutes in Group B). Postoperative complications, such as hematuria (8% in both groups), wound infection (8% in both groups), and paralytic ileus (8% in Group A, 4% in Group B), were observed, with no significant difference in the mean duration of hospital stay (8.75 ± 2.70 days in Group A and 8.75 ± 2.45 days in Group B). Follow-up at 3 weeks, 6 weeks, and 3 months revealed that 96% of patients in Group A and 92% in Group B had normal voiding, while 4% in Group A and 8% in Group B experienced per vaginal leakage of urine. The study found a slightly better success rate in Group A (96%) compared to Group B (92%), suggesting that the addition of an omental patch may enhance the repair’s effectiveness and reduce the risk of recurrence. These findings support the potential advantages of using an omental patch in VVF repair for improving postoperative outcomes and patient recovery.
