Evaluation of autologous pubovaginal sling for treatment of female stress urinary incontinence

Abstract:

Objectives: Evaluate the short-term outcomes of pubovaginal sling technique in female stress urinary incontinence (SUI).
Materials and Methods: This is a prospective study included 30 females who underwent autologous pubovaginal sling placement for SUI from January 2022 to January 2024 at Tanta Urology Department–Tanta University Hospitals. The primary endpoints of this study were the subjective improvements in the Bristol female lower urinary tract symptoms questionnaire (B-FLUTS) and cough stress test rates as an objective cure.
Results: The median age was 42.5 years old with a median body mass index of 32 kg/m2. The mean parity was 3.4±1.2, Twenty four cases with SUI and 6 cases with mixed UI. The median preoperative uroflow was 22 ml/sec, the mean operative time was 75.2 minutes. At the third (after 6 months) follow-up appointment, all of the cases completed B-FLUTS, cough test, and underwent uroflowmetry. Regarding B-FLUTS score, there was significant improvement in all symptom and bother scores from baseline preoperative levels (p= < 0.001), including the filling, voiding, incontinence, sexual and quality of life sub scores. With no significant difference between the pre- and postoperative Q-max (p=0.07). None of our cases continued to use pad and the cough test was negative in all cases. With regard to complications no patients suffered severe (Clavien III-V) postoperative complications. Regarding treatment success we had 29 patients (96.6%) cure rate.
Conclusion: In short-term follow-up, the autologous pubovaginal sling approach proved to be technically feasible, safe, and effective.
